The Treatment Coordinator (TC) partners with families and community agencies to ensure the needs of youth are adequately addressed.
The Treatment Coordinator also ensures service delivery is individualized, appropriate to need, child centered, family focused, culturally competent, strengths based, and goal driven.
The $2500 sign on bonus will be paid as follows:
$500 at hire, $500 at 30 days, and the final payment of $1500 at six months of active employment.

Documentation and Compliance prepare written summaries, Individual Service Plans and quarterly updates, discharge summaries, and program contract regarding youth and families.
ensure completion of and collect TFC parent documentation, including LODES, Medication Logs, Respite logs, copies of medical information, and any other required documentation.

Treatment Planning and Service Delivery assess each child's needs in all life domains and develop treatment plan goals.
provide ongoing advocacy and support for the child's educational needs.
Initiate and coordinate therapeutic services when appropriate.

Safety & Well Being make regular contact with each youth to assess safety and well-being.
develop an individual safety plan for each child and assist other staff, in crisis intervention to protect youth, staff, and property.

Permanency Planning confirm youth have completed an independent living assessment for life skills.
submit assessment to the appropriate individuals for scoring.
supervise any program aides that are assigned to the youth.
Working Conditions Physical Demands:
Requires vision, speech, and hearing and a moderate to high level of physical activity including; sitting, standing, walking lifting and carrying as required.
Must be able to meet the demands and complete Therapeutic Crisis Intervention (TCI).

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
